Awhile ago I became aware of a program from the Johns Hopkins School of Nursing called CAPABLE (see link below), that helps vulnerable seniors age in place at home. It has been proven to improve seniors' quality of life in many aspects while helping keep them out of hospitals and nursing homes, and thereby save expenditures for Medicare and Medicaid.
I see it as a natural fit for patients who are dual-eligible for both Medicare and Medicaid, and medical groups who are full-risk with MA or MMA. I have an opportunity that just popped up to discuss it TODAY with a regional nursing director of a large primary care group. Do any of you have any experience with CAPABLE? If so I'd love to hear from you!
