  • 1.  Care Model Pathways for Moderate Risk/Rising Risk Patients

    Good Morning,

    I'm wondering if there are any organizations that have built pathways and definitions in VBC for their moderate risk/rising risk patients.  Including resources, cadence for visits, etc.  Also, are you defining a chronic risk score or ED/Hospital Admin risk score to define these patients?

    I am also highly interested in this. One thing I have found over my 5 years in Care Management is that there are not standards around this or productivity.
