    Does anyone have chart audit tools or templates for auditing care coordinational/transitional care nursing charts? 

    We  use Epic, and we are doing a new ED discharge transitional care nurse initiative for cardiac patients. 

    I have to figure out how to measure accountability and quality....

    Any advice will help!
