  • 1.  Columbia - anyone using a "paper only" format

    As we see a number of primary care patients who would rather not communicate with us via phone (prefer portal/written communication), I am wondering if any organizations are using the portal to send a Columbia assessment in response to a positive PHQ-9 screening? The Columbia seems to be a tool created with the intention of a verbal conversation. Any information you can provide regarding using a written only version would be helpful. Thank you-

    Hello Kristin, our organization does not send the Columbia assessment via portal or hand out the assessment. For our process, we send the PHQ2 via the portal, and is sent 7 days prior to their visit. If they score 3 or higher it cascades to the rest of the PHQ questions, except for question 9. In the office during rooming, staff will get a prompt to ask the final PHQ 9 question, and if the response is positive, the screening then cascades to the Columbia assessment for staff or the provider to complete. The provider is not able to sign the encounter without the Columbia assessment being answered.
