Hi all,
I am working with a DNP student who has developed an educational intervention aimed at preventing progression of hypertension and other chronic conditions among adults with Stage 1 hypertension. She is particularly interested in exploring how ambulatory care nurses can support chronic disease prevention through patient education and lifestyle counseling.
We are seeking a clinical partner interested in helping tailor and implement this intervention within their practice setting. The project is designed to be low burden for participating clinics, and we would welcome the opportunity to adapt the program to meet local needs. We are based in metro Atlanta but could certainly deliver the intervention as a virtual module.
