Does anyone have a frailty program targeted to primary care patients? If so, what screening tool do you use? We have been using the Modified Frailty Index for our pre-surgical patients to screen for frailty, but I'm wanting to expand this program to our primary care clinics. I'm not sure if this is there is a better screening tool for that patient population. I appreciate any input that you all have!
