    Good morning,  does anyone place ID bands on their ambulatory patients?  Do you band patients who are going for procedures?  What about those coming in for testing.
    Does anyone use ID bands even for visits?
    Our organization is meeting on this topic and it would help to have some sense of what others may be doing.

    Hello, We are printing and banding all patients in our Urgent Care Clinics and our Primary Care Clinics (FM, IM, and Peds) Women's Health Clinics. we have seen success in preventing med errors as we scan the bands for medication admin and with the number of vaccines administered this is a great tool. In our urgent care settings we are administering IV hydration and meds.  We also us this as part of our 2 patient identifier process for labeling lab specimens when the band prints off it comes with several labels that meet our lab labeling requirements.

    I am in an outpatient wound care clinic. We do not band any patients. Biggest drawback is medications administration. Medications need to be manually entered as there is no scanning available. Charting is tedious, especially if your EMR is linked to a whole system if hospital medication list. Finding the correct lidocaine for example is a backward process by which you need to pick one of pages of lidocaine and then see if the NDC numbers line up.
    You can make frequent meds favorites, but with shortages manufacturers often change.

    We currently band in our urgent cares and for outpatient radiology and lab visits. But we do not band for outpatient specialty visits. We don't currently have bar code med scanning in any of our outpatient locations, but if we had the support to get that technology, we would definitely be pushing for arm bands in addition.

    We ID Band every patient in ambulatory. We use this for lab draws, medications, vaccinations, etc.

    Hello,

    We do utilize ID bands in our outpatient areas for the following scenarios:

    • All Infusions
    • Procedures requiring sedation

     

    Those have been the only two categories so far.

    We use ID bands in ambulatory oncology only.  This seems prudent due to use of high risk drugs being administered.

    I am interested in this topic and appreciate hearing what others are doing. I believe this is an important patient safety issue and will be advocating that we get back to this in our ambulatory setting.

    Yes, in our primary and specialty clinics( all part of a hospital system) we have been banding patients for the past 15 years or longer. It is a requirement at the registration desk for the patient to verify their name, spelling and date of birth and initial. We have many patients with three and four parts of their name, and also names that are very close, so checking band is a key safey step.

    We are looking into implementing this in our practices for visits, starting with primary care. Wondering if anyone has outpatient EPIC and utilizing patient ID bands?

    Thank you for bringing up this topic. Our system is getting ready to deploy bar code scanning for medications in our outpatient space.  Cerner is our EMR.  Would anyone on this forum, with current bar code scanning/arm band usage in place, be willing to share any written standard of work around their processes?  Also, is anyone tracking compliance of use - wrist band/bar code scanning? And how?

    We use Cerner as well. We weren't able to determine how to get armbands for Med admin in the office setting, but the barcode that prints on the encounter forms that follows the patient through the office visit is what we use. Staff are to continue verifying Name and DOB with each administration. It is far from perfect but definitely a step in the right direction.

    Thanks for your feedback!  Does the clinician performing medication administration reconcile the bar code from the form with the medication bar code or solely rely on name and dob? Secondly, do you track compliance of utilization of bar code scanning ?

    When administering the medication, the clinical staff will scan the patient barcode from form and verify Name and DOB. They will then proceed to scan the medication. We do use the reports from Cerner to monitor compliance. This is a far from perfect process and when we were implementing, I truly wanted to go with a barcode sticker, we could place on the patient for in the event they needed to receive any medication. We really did this in two phases - we spent the first year focusing on scanning the medication so that we are sure the patient had the appropriate medication, dose and route. Then we began focusing on the encounter form barcode. As we began tracking, I woudl personally email some of those who weren't scanning or call and speak with them. This seemed to get their attention that it was being monitored. Many of the individuals i worked with didn't have hospital backgrounds and were not used to monitoring. I have since left the role of Clinical Services Manager, but i know they are still tracking this information for each clinics monthly scorecard for clinical safety.

    We use center, but do not ID band for PCP visits. We do in procedural areas and we use barcode Med administration in those areas. We get reports on compliance for those areas with procedures. We are looking to require ID bands in all visits but don’t have the ability to do BCMA in all locations. We donate and dob

    We utilize Barcode Scanning in all 26 clinics. We only band in our infusion areas and sedation areas. All other areas, we do not scan the "patient" but do scan the meds and confirm the orders are correct. It was a learning process, but a GREAT one to have as a quality initiative in providing safe medication administration for our RNs/LPNs. We have Epic and have a data that is sent out weekly for adherence. We sustained >95% compliance consistent with LeapFrog Measures for now over a year. It is definitely a great asset for us.

    Yes, all patients are banded as a method of patient identification which includes checking the last name, first name and date of birth for all visits and hospital procedures.  The patients are also requested to review the band and initial the band that the information is correct.  I work in a hospital based setting.
