Pediatric Medical "Passport"

    Hello everyone!

    I am an ambulatory float at Connecticut Children's Medical Center and I was looking for some resources regarding Medical Passports. 

    Like pretty much all clinics, we have a wide variety of complex patients who see tons of specialties that all have different recommendations or care plans for the patients. I know there are "Medical Passports" out there but we want a more pediatric specific one that patients can bring with them to an ED or Urgent Care, etc. that would have the specialists' names and key information for that specialty so it's easily accessible. I have been drafting up some pages for different clinics that the provider would update each visit so that the family always has the most recent recommendations from the provider easily accessible. 

    I was wondering if anyone has a template for a "medical passport" that works well for their facility and if they'd be willing to share. Or even any topics or ideas on things that would be pertinent to include. I am looking to try to make sure patients are always get the most appropriate care as I feel like there can be a delay in care or even inappropriate care being given due to lack of information from the patient's specialists.
