Population Health Nurse Job Description

    My organization is currently working to create a Population Health Nurse position in our nurse clinics. Does anyone know of a job description/requirement for a Population Health Nurse. I would welcome any and all feedback.

    I manage 22 Population Health RNs and will send their job description via email for you.  We work for the Quality and Education Dept of our medical group and are responsible for CMS quality metrics directly and through insurance payers.  I'll send today and you can contact me for additional information if needed
