    Hello AAACN members,

    I am looking to see for specialty practices what support positions are in place. Do you have MSWs, CHWs, etc.? What does this look like for your organization?

    HI Ashley,

    Our outpatient social work department provides services to all specialty practices. You can request these services through a shared referral with RN care management entered in EPIC. Our social work team is mainly composed of LMSWs.

    Hi Tricia,

     

    Do you have social work embedded in any practices or is it centralized and they work from referrals coming in across ambulatory? We have MSWs in primary care but only a handful in specialty clinics. Our specialty offices are requesting social workers, but I think a CHW and escalation pathway to MSW if needed may be more optimal.
