  • 1.  Transcutaneous Bilirubin

    Hi,

    Does anyone use transcutaneous bilirubin devices in their clinics? If yes, where do you chart the results? If you use Epic, that would be even better. This is a new implementation for us, and there is a lot of back and forth whether we chart it with our other POCT results, in VS section, procedures. Any help would be great.

    Hello,

    We document the results using the enter/edit function in Epic, which places the results in the labs section and allows them to easily be compared to other results. 

    We implemented the TCBs over the last year so feel free to reach out if you have additional questions.

    Sorry, just seeing this now. No, it is not considered a "lab" and thus does not fall under CLIA regulations.

    Awesome, thanks! That's where I want it documented too. Then, it was feared that if we did it there, we had to build competencies around it bc of it being a POCT. thanks for your input!

    Good morning Sarah! 

    Our pediatric clinics are piloting this now, and documentation was an area where there was quite a bit of conversation. We utilize Epic as well. 
    Currently our providers are the sole operators of this technology, and they have access to the organization's flowsheets used in our Labor & Delivery. At this time, they are able to document and trend TcB levels through this means - certainly for newborns birthed at our facility, and I believe (although this I am not 100% certain of) also for those who were not. The flowsheet allows for both tracking and links to the approved interpretation tool. Had this not been an option, or if our pilot finds it suboptimal in some way, we may ask our IT for a build into our Rooming tab. The technology we purchased also supplied a printable tracking curve and protocols for utilization, which could be another option.
