Hello everyone,
I'm currently gathering insights on process improvement or project ideas for addressing Social Determinants of Health (SDOH). I'm seeking innovative ideas and best practices to that are aimed at improving healthcare outcomes by addressing issues within the 9 Pillars of social risk domains ( food security, utilities, housing, digital needs, social isolation/loneliness, transportation, employment, legal, and education). Specifically, I'd love to hear about successful projects or approaches you've implemented to tackle SDOH in your organization.
Key areas of interest include:
- Strategies for identifying and addressing SDOH within patient populations.
- Effective community partnerships or collaborations that have made a significant impact.
- Use of technology and data analytics to track and improve SDOH-related outcomes.
