  • 1.  Patient transport to ED

    We have several ambulatory offices attached to our Hospital. Offices are stating when patients need a higher level of care (i.e. the ED) and are stable they are putting them in a wheelchair and transporting them to the ED with one nurse. The nurse has no emergency equipment and it's a very long trip walking to the emergency room. I recommend sending all patients requiring a higher level of care via ambulance. Please share your process or recommendations.

    Hi, we have an Incident Management Team (IMT) that arrives to help in the clinic areas when the page goes out. This includes members of respiratory therapy (RT), house supervisor (NAC), security, nurses and other. If stable, a couple of nurses/providers, the NAC, RT and security will take them to the ER. Equipment is included. If needed, we can upgrade at any time and the Rapid Response Team arrives from the ER and will stabilize/transport.

    Carrie,

    We are an outpatient surgery center. All patients needing a higher level of care are required to be transferred via ambulance. If they are stable and they refuse to go by ambulance, they will need to sign out AMA. Our ER department is across the parking lot.

    Great question and one that we have wrestled through as well.  We have decided that all unstable patients are sent by ambulance to the ED regardless of if the distance is a short walk, down the elevator etc.  Our ambulatory team is not ACLS, we do not have equipment that we transport the patient with.  We did ask the hospital if they would come to us.  They also affirmed our need to call for an ambulance.

    If we are transferring to a higher level of care we call emergency dispatch who transfers the patient for us.

    Greetings,

    Our nurse leadership team is debating if there is evidence to continue to recommend clinical staff draw up medications in front of the patient prior to administration to limit the chance of medication error. Can you please share your process for safely preparing medications?
