Good Morning - My organization is currently trying to create an "intake office" for patients new to the organization looking to be seen by a PCP. We have tried multiple iterations of this in the past including a nurse "appointment" either by phone or in person to interview the patient and data entry into the chart, providing scheduling with an electronic algorithm to ensure patients were appropriately scheduled and their needs met, etc. These have came with their own benefits and areas of opportunity. Looking to see if anyone has a process they would be willing to share. Our organization has 40+ primary care clinics spread over 3 states, ideally this "intake office" would outreach to all of these patients to ensure their needs are met and their chart is built prior to seeing the clinician.
