  • 1.  Vital Signs in Clinics

    I am reaching out to see what my peers are doing regarding Vital Signs in your Ambulatory clinics. It is a given that all Primary Care/Family Medicine clinics perform VS as our basic screening for each patient, but what about your Specialty clinics? I get pushback from providers stating VS are not necessary in the Specialty clinics. I disagree stating that it gives us an opportunity to capture potential issues with patients. What are your thoughts?

    Hi Michael, 

    This is something that I have been trying to integrate into my clinic, as well. Interestingly, VS seem to be a relative standard for primary care provider visits but not for nurse specific visits (eg: vaccinations, teachings, etc).

    At our OB, MAT, and Dental sites, we do vital signs at all of their visits alongside our Family Practice visits.  We do not have ortho, or other specialties associated with our practice, so I have nothing to offer in that sense.  

    Our nurse visits, it depends on what kind of visit it is.  If we are doing a birth control visit, we do Ht, Wt, BP,  but no temp or pulse, or respirations  (most of us do a full set of vitals anyways).

    For our phlebotomy (lab draw) nurse visits, we do not do any vitals unless the patient condition warrants.  95% of the time we don't 

    Our vaccination nurse visits we do a full set of vitals.

    We obtain vital signs as well as height/ weight in our orthopedic clinic.  It can impact surgical decisions because of BMI and where patients can be accommodated for imaging.  There was some pushback initially because of how to address out of range vital signs but it is now a  routine part of rooming patients.
