My organization has a DM Case Management program that focuses on our members with diabetes who have an A1c >8. Each member is assigned to either an RN (CDCES certified) or a pharmacist as their case manager. Both roles are able to titrate/manage insulin and oral medications based on protocols.
We are looking to further optimize the role of the RN in this program, and I would love to hear from other organizations that may have similar programs. Some of my questions include:
- What roles are part of the Case Management team?
- If there are different roles, how is the member assigned?
- How is care provided (virtual/in-person/hybrid)?
- What does the Case Management team handle that is not typically handled by the rest of the primary care nurses?
- What is the typical cadence of visits and duration of the program?
- Any data showing outcomes of the program.
